Violations: Observed nonfood-contact surfaces constructed of unapproved materials. Cardboard lining shelves near sink. Nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning shall be smooth and durable.

Equipment and/or components are not maintained in good working order. Walk in cooler is leaking from the roof. Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. Correct By: 22-Aug-2017.

Observed accumulation of soil residue on nonfood-contact surfaces. Shelves have accumulation of grease. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.

Comments: Have level 2 certified employee, vomitus cleanup, employee health policy.

Note: Walk in leak must be repaired. A reinspection is scheduled for above to ensure it has been properly repaired.
