TriHealth, CVS are latest to partner

TriHealth announced today a new clinical collaboration with CVS/Pharmacy and Minute Clinic to increase access to care and improve medication adherence for patients, according to the health system.

“TriHealth and CVS Health have established a clinical affiliation given our mutual goal of improving the lives and health outcomes of the patients we serve,” said Steve Mombach, vice president of ambulatory services for TriHealth, in a release. “Through this collaboration, we believe our patients will have enhanced access to pharmacy care and coordinated, primary care support.”

Through these clinical affiliations, CVS Health will provide prescription and visit information to the participating health care organizations through the integration of secured electronic medical record systems. TriHealth patients will also have access to clinical support, medication, counseling, chronic disease monitoring and wellness programs at CVS Pharmacy stores and MinuteClinic.

Many patients often struggle with prescription management, especially those with multiple medications due to chronic disease, according to TriHealth.

To help improve health outcomes and lower medical costs, the collaboration with CVS Health will support providers’ care plans and the communication of critical patient and pharmacy care information.

“Our top priority is our patients. As a Patient-Centered Medical Home, primary care physicians view themselves as the quarterback for our patients’ care,” said Dr. Diane Dolensky, medical director of TriHealth Physician Practices, in a release.

CVS/Pharmacy currently has more than 7,800 retail pharmacy locations across the U.S. where CVS pharmacists provide counseling to patients to help with medication adherence.

In addition, MinuteClinic provides timely, affordable and high-quality walk-in health care. There are more than 900 MinuteClinic walk-in medical clinics available at CVS/Pharmacy retail stores. MinuteClinic locations are open seven days a week.

“When the immediate needs of our patients lead them to seek care outside of our practice, we want to provide options where we have quality, collaboration and continuity of care while still knowing we’re all on the same team,” Dolensky said.
