Schwarber, 28, hit .266 with 32 home runs and 71 RBI combined this season for the Nationals and the Red Sox.
In June, Schwarber hit 16 homers in 18 days, including 12 in a 10-day stretch. He was named National League Player of the Month and made the NL All-Star team.
He strained his hamstring in early July, and was traded to the Red Sox just before the deadline. He returned from the injury on Aug. 14 and hit .291 in 41 games for Boston.
Schwarber spent six seasons with the Cubs before signing a one-year deal with the Nationals prior to this season.
The Red Sox open an AL Divisional Series vs. the Tampa Bay Rays on Thursday night.
