Middletown is believed to have gotten its name from its founder, Stephen Vail. One local historian believes that Vail named Middletown after his hometown of Middletown, New Jersey. Another believes that it was named because it was the midway point of navigation on the Great Miami River.

Middletown contains a small, municipal airport called Hook Field that no longer serves commercial airlines just general aviation. A regional campus of Miami University is located in Middletown. One of the nation's top 100 hospitals, the Atrium Medical Center, is also located in Middletown.

Several well-known figures come from Middletown including the late Hawthorn Heights guitairist, Casey Calvert, and former NFL Pro-Bowl and All Decade player and analyst, Cris Carter. Middletown is also home to the Ohio Hot Air Balloon Challenge and many other local festivals.
